Glycosaminoglycan (fast-moving heparin plus dermatan sulphate)
Sulodexide
Endothelial-tropic glycosaminoglycan that supports antithrombin- and heparin cofactor II–mediated anticoagulation at relatively low systemic anticoagulant intensity, and interacts with the vascular glycocalyx.
Primary Mechanism of Action
Clinical / Scientific
Sulodexide is a heparinoid mixture. It potentiates antithrombin (anti-Xa/IIa) and heparin cofactor II (dermatan-related anti-IIa), and has been shown to interact with endothelial glycosaminoglycan layers. Anti-inflammatory and matrix-related effects (including MMP modulation in some studies) are adjunctive mechanisms under investigation rather than a licence to claim broad vascular “restoration”.
Pathway Targets
Antithrombin / HCII
Scientific explanation
Serpin-mediated inhibition of thrombin and factor Xa.
Endothelial glycocalyx
Scientific explanation
Glycosaminoglycan replenishment / interaction.
Inflammatory signalling
Scientific explanation
Reported downstream effects on endothelial activation in studies.
Pathway Convergence
Clinical / Scientific
Target → pathway → downstream effect → biological consequence. This is a mechanistic map, not a treatment claim.
Surface anticoagulation plus barrier support
Target to downstream effect: Serpin potentiation → Reduced thrombin activity → Glycocalyx interaction → Lower endothelial activation in studied settings
Mechanistically Relevant Repurposed & Adjunctive Applications
Chronic venous disease / selected vascular labels
EstablishedMechanistic rationale
Approved in some jurisdictions for chronic venous disorders and selected vascular indications. Labels are regional.
Diabetic nephropathy and endothelial research
InvestigationalMechanistic rationale
Glycocalyx and albuminuria studies are investigational relative to many markets.
Mechanistic Application Matrix
| Biological Target | Mechanism | Potential Relevance | Evidence Level |
|---|---|---|---|
| Thrombin / Xa via serpins | Potentiation of AT/HCII | Haemostasis | Established mechanism |
| Glycocalyx | GAG interaction | Endothelial barrier | Investigational |
Mechanistic Interaction Considerations
Additive anticoagulant effects with heparins, DOACs, antiplatelets, and NSAIDs are mechanistically plausible. This is not a complete bleeding-risk monograph.
In Plain Language
Sulodexide is a mix of heparin-like sugars. It gently boosts the body’s own clot-controlling proteins and can interact with the slippery coating on blood-vessel lining cells.
